CASES is a New York City organization working at the forefront of the intersection between behavioral health and involvement in the criminal legal system. To advance our mission of innovative services that reduce crime and incarceration, promote recovery and rehabilitation, and create opportunities for success in the community, CASES is collaborating in a new community-based reentry network to serve individuals released from Rikers Island. The Reentry Specialist will engage a small caseload of people released from jail providing needed supports in the community. The Reentry Specialist will facilitate smooth reentry transitions and will assist clients to engage in the services that will address reentry needs, including linkage and retention in the CASES Nathaniel Clinic Certified Community Behavioral Health Clinic (CCBHC).


  • Provide short-term reentry supports to people released from Rikers Island delivering community-based services
  • Complete mobile community-based work including traveling to Rikers Island to complete day-of-release warm handoffs, to assist clients with food, clothing, income support, health insurance, housing and shelter, and other immediate needs
  • Deliver outreach and engagement services that offer companionship and raise pathways to needed supports and services
  • Coordinate with the Nathaniel Clinic CCBHC treatment, rehabilitation, and peer staff to facilitate the successful engagement of clients in treatment services
  • Deliver community-based mental health assessment and crisis intervention services consistent with outpatient mental health standards to support ease of treatment access and improved engagement in care during the client’s transition from jail
  • Successfully connect clients to longer term community support services, such as education, vocational training, employment, and other supports utilizing the reentry network providers
  • Assist clients to connect to natural supports that enhance the quality and security of community living
  • Document all services in a comprehensive and timely manner in the case record, including progress notes, referrals and linkages, discharge notes, and mental health assessments and crisis services
  • Attend relevant trainings and meetings, especially related to re-entry, behavioral health/criminal legal system, and treatment
  • Collaborate professionally and effectively with the reentry network lead agency and providers
  • Provide reentry supports and services in a strengths-based, recovery-oriented, trauma-informed, anti-racist and culturally humble manner
  • Ensure compliance with all applicable confidentiality standards, policies, and laws
  • Any other duties as required by the supervisory and senior staff.


  • New York State license LMSW/LMHC, preference to individuals with lived criminal legal system experience
  • Previous experience working with individuals who have mental illness and/or substance use disorders and those impacted by the criminal legal system
  • Bilingual Spanish-speaking a plus.
  • Strong interpersonal and communication skills
  • Tolerance for ambiguity in developing a new role
  • Persons with lived experience in the areas of mental health, substance use, and criminal justice are encouraged to apply
